Click here to complete the student clinical request form. The form will be emailed to [email protected] and must be received four weeks before the start of the clinical rotation. It is the student's responsibility to complete the form. Forms submitted on behalf of the student will not be accepted.
Any student clinical request form received less than 8 weeks before the start date will result in a delay in the clinical start date.
A student/non-employee workday request will be submitted for on-boarding. This request consist of three steps.
Step 1. The request is started by the Student Clinical Manager (CHI St. Vincent Sponsor) then sent to the student (non-employee).
Step 2. The student (non-employee) will sign the confidentiality acknowledgment agreement and forward the email to their clinical coordinator at the school for completion.
Step 3. The school will attest that the school has proof of immunizations by indicating either a yes or no that the immunizations are complete.
When all parties have completed the Request, the school will submit it to Human Resources (HR). A confirmation email will be sent to all parties upon completion. HR's service goal is three business days to enter the non-employee into workday. Workday will send an auto -generated email to the Student Clinical Manager.
Student Epic access will be granted as requested.
If you have an Evidence-based Nursing Project or Nursing Research Study you plan to conduct at CHI St. Vincent Hot Springs, please report your topic to the Hot Springs Research Task Force. All Nursing Research Studies conducted at Hot Springs must be reviewed by the task force. The task force would also like to offer assistance to students doing nursing research if needed.
