At CHI St. Vincent, we make the healing presence of God known by improving the health of the people we serve, especially those who are vulnerable. Every three years, we conduct a Community Health Needs Assessment.
It only takes a few minutes to answer the questions in this survey so that we might better understand your experience with health care and socio-economic issues that affect our communities. Your perspective as a member of our community is extremely valuable. Your answers are confidential and will be combined with other surveys, interviews, and focus groups to guide much of our community outreach efforts for the next three years.
